Support Plan

We are committed to an individualised service by implementing Individual Service User Contracts of Care, Initial Assessment and Personal Care Plans. We encourage Independent Living Skills, Makaton Language Skills and Self-Help Skills to name but a few.

 

We maintain Positive Family and Independent Advocacy links. We encourage College/Further Education and Work Placements. In addition to this SFI participates in a wide range of Activities in the Community.

After short, medium and long term goals are identified, the Action Plan is then formulated for our service users.
